The brand is present in AI answers at a 50.2% raw mention presence rate, but that presence converts into a genuine recommendation less than one time in five. Executive Summary

Elevance Health is visible in AI answers but under-recommended. The benchmark records a raw mention presence rate of 50.2% in October 2026, meaning the brand appears in roughly half of qualified observations. Its valid recommendation coverage is 18.2%, the lowest of the ten tracked brands, and its top-three recommendation rate is 7.8%. The gap between presence and recommendation is the defining feature of the brand's position in this measurement.

The mention mix is heavily neutral. Of 245 present observations, 151 are neutral, 93 are positive, and 1 is negative, producing a net sentiment score of 0.3755, also the lowest in the tracked set. Elevance Health is being referenced as context more often than it is being recommended as a choice.

The strongest cluster signal is also the only cluster with qualified data. All 488 qualified observations fall into the Best Vision Insurance Plans cluster, where Elevance Health records a 7.8% top-three rate and no rank-one placements. The Pricing and Value and Multi-Brand Comparison clusters carried zero qualified observations in October 2026, so the benchmark cannot yet answer cost or head-to-head comparison questions at the qualified level.

The strongest platform signal is Google AI Overviews, where Elevance Health records a 13.6% valid recommendation coverage rate and a 56.5% raw mention presence rate. The weakest platform signal is Copilot, where valid recommendation coverage falls to 35.8% and top-three placement to 5.7%. ChatGPT records the lowest positive visibility rate of any platform at 13.6%.

The clearest gap is conversion from reference to recommendation. Kaiser Permanente converts 81.8% presence into 55.5% coverage and a 40.0% rank-one rate. Blue Cross Blue Shield converts 93.7% presence into 65.4% coverage and a 50.8% top-three rate. Elevance Health converts 50.2% presence into 18.2% coverage and a 7.8% top-three rate. The brand is in the room but not on the shortlist.

What Elevance Health Is Winning

Questions This Section Answers

  • How much did Elevance Health's recommendation coverage improve between July and October 2026?
  • How does Elevance Health's top-three rate compare with other tracked brands?

The evidence-backed wins are narrow. Elevance Health improved its valid recommendation coverage from 10.3% in July 2026 to 18.2% in October 2026, a 7.9 percentage point gain that the benchmark classifies as a move beyond normal variation to the upside. Raw mention presence rose 7.5 points over the same window.

The brand records no negative framing at scale. Of 245 present observations, only 1 is negative, and the net sentiment score of 0.3755 is positive on the benchmark's 0 to 1 scale. That is a framing-quality signal, not a customer sentiment signal, but it means AI systems are not describing the brand in cautionary terms.

Elevance Health also records a 7.8% top-three rate, which places it above Cigna at 5.3% and above no other tracked brand. The brand is not absent from recommendation sets. It is thinly represented in them.

Where Elevance Health Has the Clearest AI Visibility Gaps

Questions This Section Answers

  • Why does Elevance Health's mention presence fail to convert into recommendations?
  • Where does Elevance Health lose first-position placements to Kaiser Permanente and Blue Cross Blue Shield?
  • Which platforms show the widest positive-visibility gap between Elevance Health and category leaders?

The primary gap is recommendation conversion. Elevance Health appears in 50.2% of qualified observations but receives a valid recommendation in only 18.2% of them. Kaiser Permanente appears in 81.8% of observations and receives a valid recommendation in 55.5%. The two brands are separated by 31.6 points of presence and 37.3 points of coverage, which means Kaiser Permanente converts its visibility into recommendation credit at a materially higher rate.

The second gap is first-position placement. Elevance Health records a 0.0% rank-one rate across all 488 qualified observations. Kaiser Permanente records 40.0%, Blue Cross Blue Shield records 10.0%, and UnitedHealthcare Vision records 11.7%. Elevance Health is not being named first in any qualified observation in the October 2026 set.

The third gap is neutral framing concentration. Elevance Health carries 151 neutral mentions against 93 positive mentions, a ratio of roughly 1.6 to 1. Blue Cross Blue Shield carries 128 neutral against 329 positive, a ratio of roughly 0.4 to 1. Elevance Health is more likely to be described than endorsed.

Platform-level gaps reinforce the pattern. On ChatGPT, Elevance Health records a 13.6% positive visibility rate against Kaiser Permanente at 77.3% and Blue Cross Blue Shield at 70.5%. On Google AI Overviews, the brand records a 13.6% positive visibility rate against Kaiser Permanente at 53.9%. The brand's strongest platform, Google AI Overviews, still places it well behind the category leaders.

Biggest Opportunity

Questions This Section Answers

  • Which prompt cluster offers the clearest path from mention to recommendation for Elevance Health?
  • What is missing between Elevance Health's retrieval layer and its recommendation layer in the Best Vision Insurance Plans cluster?

The clearest path from reference to recommendation runs through the Best Vision Insurance Plans cluster, the only cluster with qualified observations in October 2026. Elevance Health records a 7.8% top-three rate and a 4.1 average recommended rank in that cluster, against Kaiser Permanente at 47.5% and 1.7. The cluster contains prompts such as "What is the best medical insurance to get?" and "Which health insurance is best?" where the brand is present but not selected.

The opportunity is to move Elevance Health from a named option into a named recommendation within that cluster. The benchmark shows the brand already appears in roughly half of qualified observations, so the retrieval layer is functioning. What is missing is the recommendation layer: the owned answer content, comparison framing, and citation support that lead AI systems to place a brand inside a shortlist rather than beside it.

Competitive Landscape

Questions This Section Answers

  • Where does Elevance Health rank against the other tracked brands on top-three and rank-one rates?
  • How does Elevance Health's average recommended rank compare with its inclusion frequency?

Elevance Health ranks ninth of ten on top-three rate and holds the lowest sentiment score in the set. Its average recommended rank of 4.1410 is mid-pack, which indicates that when the brand does receive rank credit, it lands in the middle of the list rather than at the bottom. The constraint is frequency of inclusion, not position within the list.

What CiteWorks Studio Would Do Next

Phase 1: AI Market Discovery Audit Map every qualified prompt where Elevance Health appears without recommendation credit, and identify which competitor is recorded in the recommendation slot the brand is losing.

Phase 2: Recommendation Readiness Plan Prioritize the Best Vision Insurance Plans cluster and the Google AI Overviews and ChatGPT surfaces where the brand's positive visibility rate is furthest behind the category leaders.

Phase 3: Owned Answer Layer Buildout Build owned pages that answer the specific high-intent questions in the cluster directly, so AI systems have a clear, retrievable source that names Elevance Health as a recommended option rather than a referenced one.

Phase 4: Citation / Authority Layer Development Strengthen the public evidence layer around the brand, since no tracked brand's own domain appears among the top ten cited domains and the citation environment is dominated by comparison and personal-finance publishers.

Phase 5: Monthly AI Visibility and Recommendation Tracking Track coverage, top-three rate, rank-one rate, and sentiment month over month to confirm whether the conversion gap between presence and recommendation is closing.

Why This Matters

Questions This Section Answers

  • Why is a neutral AI mention not the same as a buyer shortlist placement for Elevance Health?
  • What layers of correction does the benchmark identify to close Elevance Health's presence-to-recommendation gap?

AI presence alone is not a business outcome. Elevance Health appears in half of qualified AI answers, but a mention that does not convert into a recommendation does not place the brand on a buyer shortlist. The benchmark separates presence from recommendation for exactly this reason: a neutral reference, a cautionary mention, and a competitor-displaced mention are not equal signals.

The next move is targeted correction of the prompt, page, and citation layers. The brand needs owned answers that match the questions buyers actually ask, framing that positions Elevance Health as a recommended choice rather than a named option, and citation support that makes those answers easy for AI systems to retrieve and synthesize.

Sentiment Score

Sentiment Score = (positive mentions × 1 + neutral mentions × 0 + negative mentions × -1) / total mentions

For Elevance Health in October 2026: (93 × 1 + 151 × 0 + 1 × -1) / 245 = 0.3755.

This matters because unclassified mention counts are misleading. A brand with 245 mentions sounds visible until the mentions are classified and 151 of them turn out to be neutral references rather than endorsements. Share of voice is a diagnostic metric, not a business KPI. A positive recommendation, a neutral reference, a cautionary mention, and a competitor-displaced mention are not equal, and counting all mentions as wins is bad measurement. Classified framing is required before interpreting AI visibility, and Elevance Health's classification shows a brand that is described far more often than it is recommended.

Methodology

  1. This report is a benchmark-based analysis of Elevance Health's position in the October 2026 Health Insurance AI Market Discovery Index. It is not a client implementation case study.
  2. The reporting window is October 2026, with comparison points from July 2026, August 2026, and September 2026 where the benchmark provides them.
  3. Six AI and search surface families were tracked: ChatGPT, Copilot, Gemini, Perplexity, AI Overviews, and AI Mode. All six recorded at least one qualified observation in October 2026.
  4. The October 2026 measurement began with 800 source prompt-surface observations and 552 unique questions. Of those, 520 were judged relevant and 280 irrelevant, producing 488 qualified benchmark observations.
  5. The competitor universe contains ten tracked brands: Aetna Vision Preferred, Ambetter (Centene), Blue Cross Blue Shield, Cigna, Elevance Health, Humana Vision, Kaiser Permanente, Molina Healthcare, Oscar Health, and UnitedHealthcare Vision.
  6. Three public high-intent clusters were defined: Best Vision Insurance Plans (consideration stage), Vision Insurance Plan Comparisons (evaluation stage), and Vision Insurance Pricing and Costs (decision stage). Only the first cluster carried qualified observations in October 2026.
  7. Stage 0 extraction retains the query, AI or search surface, answer, brand outcome, recommendation placement, sentiment, and, where exposed, citations or attributable evidence sources.
  8. A mention is counted when Elevance Health appears anywhere in a qualified AI response, regardless of whether it is recommended.
  9. A valid recommendation is counted only when the dataset marks the brand as a genuine, usable recommendation. Neutral, cautionary, comparison-anchor, and listed-only mentions are not counted as valid recommendations.
  10. Top-three rate and rank-one rate are calculated against the 488 qualified observations. Average recommended rank covers rank-eligible recommendations only.
  11. The benchmark does not measure market share, attributable sales, organic-search ranking, social mention volume, or private and sponsored channels. A metric movement alone does not establish causality.
  12. The tracked set for October 2026 includes vision-specific brand variants for Aetna, Humana, and UnitedHealthcare that were not measured under those names in July 2026. Where a brand was not measured under its current name at baseline, its July 2026 figure is shown as 0.0% for reference only and is not a movement result.

See How AI Is Recommending Your Brand

The benchmark shows where Elevance Health stands in AI recommendations across the Health Insurance category. A company-level AI visibility audit maps the specific prompts, surfaces, competitors, and evidence sources behind those numbers into a prioritized strategy, so the brand can see exactly which questions it is losing and where its next recommendation gain is most likely to come from.
